GP Referral Scheme


A GP Exercise Referral Scheme is a program where GPs or other healthcare professionals refer individuals to tailored exercise programs. These programs are designed to help improve health, manage specific conditions, and enhance quality of life through regular physical activity. The referral is often aimed at those who have medical conditions or lifestyle factors that make it difficult to engage in exercise on their own.

Who is this scheme for?

GP Exercise Referral schemes are suitable for individuals who:

  • Are living with long-term medical conditions such as diabetes, hypertension, arthritis, cancer, amputation, obesity or parkinson’s disease.
  • Are at risk of chronic diseases due to lifestyle factors e.g sedentary lifestyle.
  • Are recovering from surgery or illness and need structured rehabilitation.
  • Are looking for expert guidance to safely increase their physical activity.
  • Have mental health conditions like depression or anxiety that may benefit from exercise.

How does it work?

1.Consultation with your GP
If you’re experiencing health concerns that could benefit from exercise, talk to your GP or healthcare professional. They will assess your health, discuss your needs, and determine if an exercise referral program is appropriate for you.
 
2. Referral to an Approved Exercise Provider
Once referred, you’ll be directed to a certified exercise professional, typically at a local leisure center, gym, or fitness facility of your choosing.

You will attend a one-to-one consultation with our GP referral instructor.
Our professionals will design a personalized exercise plan based on your medical history, abilities, and fitness goals.
Referrals are 12 weeks long and include a personalized exercise plan, gym membership/entry, check ins, support and supervised group gym sessions with your GP referral instructor. 

3.Personalized Exercise Plan
Your exercise program will include safe and effective activities tailored to your needs.
You’ll be supported and monitored throughout your program to ensure progress and adjust as necessary.
You will attend supervised group sessions
 
4.Ongoing Support and Monitoring
Regular follow-ups are scheduled to track your progress and provide motivation.
The program can be extended if necessary, with continued support to help you maintain a healthy lifestyle long-term.
 

5. Completion Support and Progression
Upon completion of your 12-week GP exercise referral, you can:

  • Continue group sessions with repeat exercise referral.
  • Continue independantly with your gym programme.
  • Be eligible to receive discounted gym membership.
  • Advance to personal training sessions with your GP exercise referral instructor.
  • Purchase a new training programme.

ProActive Activity Accreditation

We are proud to be accredited by the ProActive Physical Activity on Referral Scheme,
 which helps people living with health conditions to become more active in a safe and
 structured environment. Health and leisure professionals can refer individuals to us for
 support with improving their overall health through targeted exercises prescribed by
 our highly qualified exercise on referral fitness professionals.

Before accessing any activity, an individual is health screened and inducted into the
 centre, with a personalised 12-week programme tailored to the needs of their
 condition(s), including strategies to improve or manage symptoms and specific
 rehabilitation goals.

 On completion of the programme, the referring health or fitness professional is notified
 of progress and outcomes, and individuals have a physical activity plan designed with
 them which includes some next steps.
